and tax clearance paper from the regional finance bureau valid for
the year 1998 E.C.

Health Office now invites wax-sealed bid from eligible bidders for
the furnishing of the necessary labor, materials, equipment for
the construction & completion of Chronic Diseases Follow-Up Clinic
at different areas located in the zone as listed below:

- A complete
set of bidding document may be purchased by any interested
eligible bidder on submission of a written application to the
above office & up on non-refundable payment of ETH Birr 200 per
document, after this announcement until the 30th day
from West Wellega Zone Health Office.
- Bids shall
be valid for a period of 120 days after bid opening & must be
accompanied by a bid security at least 1% of the bid amount per
site in an acceptable CPO, unconditional Bank Guarantee in sealed
envelope on or before at 6:00 A.M. Local time to the zonal office.
- Bid will be
opened in the presence of bidders or their legal representatives
who choose to attend on the next working day at 8:00 P.M. local
time in the zonal administration office.
- The
construction works shall be completed within 180 calendar days
from the commencement of the works.
- The office
reserves the right to reject any or all bids.
