registration certificate, VAT registered certificate, Clearance
given by Oromiya Finance & Economic Development taxes and bidders
should also submit experience letters.
1. West
Arsii Zone Health Office invites wax-sealed bids form financially
and technically capable bidders to furnish the necessary materials,
Labor and equipment for construction and completion of Habano
nucleus Health center at Kokosa Woreda.
2. Bidding
document and additional set of document may be purchased on
submission of written application to Arsii Zone Health Office for
non-refundable fee of Birr 200 (Two Hundred Birr) on working days
and hours.
3. Bidders
are requested to fill all necessary details in bidding documents and
are also instructed to fill, sign and seal each page of bid
documents, failure to follow this instruction may cause automatic
rejection.
4. The
bid document shall be filled and completed in one original and two
copies and returned back in separate wax-sealed envelopes labeled
“Original “ and “Copy” copy must be “Machine Photo Copy”.
5. All
bids must be accompanied by bid security of at least 1% of bid
amount in acceptable CPO Valid for 90 calendar days and must be
delivered to West Arsii Zone H/Office on or before May 8, 2008 at 10:00 A.M.
6. Bids
will be opened in presence of bidders or their authorized
representatives who choose to attend in the same date at 2:00 P.M.
O’clock West Arsii Zone Administration Office.
7. The
Construction works shall be completed within a maximum of 180
calendars days form commencement of works.
8. The
employer reserves the right to reject and or all bids.
