To all
contractors of category GC/BC 6 and above with license valid for the
year 2000 E.C. Renewed registration certificate, VAT registered
certificate, Clearance given by Oromia Finance and Economic
Development Bureau which is showing that the bidder has paid all
required government taxes and bidders should also submit experience
letters.
1. West
Arsii Zone Health Office invites Wax-sealed bids form financially
and technically capable bidders to furnish the necessary materials,
Labor and equipment for construction and completion of Allummaa
uncleus Health center at Dodolaa Woreda.
2. Bidding
document and additional set of document may be purchased submission
of written application to West Arsii Zone Health Office for
Non-refundable fee of Birr 200 (Two Hundred Birr) on Working days
and hours.
3. Bidders
are requested to fill all necessary details in bidding documents and
are also instructed to fill, sign and sealed each page of bid
document, failure to follow this instruction may cause automatic
rejection.
4. The
bid document shall be filled and completed in one original and two
copies and returned back in separate wax-sealed envelopes labeled
“Original” and “Copy” copy must be “Machine Photo copy”.
5. All
bids must be accompanied by bid security of at least 1% of bid
amount in acceptable CPO Valid for 90 calendar days and must be
delivered to West Arsi Zone H/Office on or before Feb. 11/08 at
12:00 O’clock (noon).
6. Bids
will be opened in presence of bidders or their authorized
representatives who choose to attend in the same date at 2:00 P.M.
O’clock West Arsii Zone Administration office.
7. The
construction works shall be completed with in a maximum of 180
calendar days from commencement of works.
8. The
employer reserves the right to reject any or all bids.
