1. Arsi
Zone Health Department invites wax-sealed bids for eligible bidders
for furnishing the necessary building materials and labour for
construction of Harabi, Magna-Legebuna, Dheleta-Cheffa, Lafto,
Kiyasho Sirbo, Lode-Lemefo, Gersa, Sole, Dimma, Addis-Hiwot,
Weleargi, Shune, Wabe-Kebena, and Semar Nucleus Health Ceneters at
D/Tijo, Chole, Aseko, Gololcha, Z/dugda, L/Bilbilo, Sire Sude,
Gobessa, Gunna, Jeju, Hetossa, Munessa, H/Wabe, Sude,Woreda
respectively.
2. Bidding
document may be purchased at the above mentioned Dep’t of Planning
and program service up on submission of written application and
non-refundable fee of 200 birr for each set during the working days
and before 4:00 p.m. starting for the last consecutive ten working
days from bid opening.
3. All
bids must be accompanied by a bid bond of 1%in the form of CPO
(Cashier Payable Order) and valid for 120 calendar days after the
date of bid opening.
4. All
eligibility certificates from the Federal Ministry of Infrastructure
or Regional Works & Urban Development Bureau, renewed license for
the year, VAT registration, TAX payer registration and clearance
given by Oromia Economic and Finance Bureau delivered during
registration original with their copies.
5. Bid
shall be submitted in one original and two copies with each separate
wax-sealed envelope.
6. Bid
bond should be in separate envelope with company profile and each
separately bid documents submitted all in one compiled wax-sealed
envelope except the company profile wax-sealed separately in tender
box.
7. Recent
one year original financial statement report from recognized bank
should be enclosed in company profile.
8. All
bids must be deposited in the tender box prepared at the Zone Health
Department for this purpose at or before 9:00 a.m. on December, 7/2009.
9. Bids
will be opened in the same day in presence of bidders and/or their
official representatives who wish to attend on December, 7/2009 at
10:00 a.m. at Zone Administration Hall.
10. Construction
work should be completed within a maximum of 270 calendar days
including mobilization time.
11. The
Department reserves the right to reject or accept any or all bids.
Failure to comply any of the criteria
written above will result in automatic rejection.
